Kansas State High School Activities Association requires a Pre-Participation Physical Evaluation (PPE) annually and shall not be taken earlier than April 1 preceding the school year for which it is applicable.
You will now upload Preparticipation form using the Bound app. You WILL NOT bring your forms into the athletic office. More instructions will come from the district.

In addition to the physical forms, athletes and their parents/guardians are required to complete the High School Packet found online in Skyward. These forms must be completed by the student athlete's first day of tryouts. If either parent/guardian or student have issues with Skyward, please contact the counseling office.
